Leg Length Discrepancy (LLD) refers to a condition in which one leg is shorter than the other. It is a common finding in both children and adults and may result from a variety of causes. Some individuals are born with structural differences in leg length, while others develop LLD following fractures, surgery, hip disorders, growth disturbances, infections affecting bone development, or certain neurological and musculoskeletal conditions. In some cases, the legs are equal in length, but muscle imbalance, joint contractures, pelvic asymmetry, or abnormal biomechanics create the appearance of unequal leg length—known as functional leg length discrepancy.


The clinical significance of LLD depends on both the magnitude of the discrepancy and the individual’s symptoms. Small differences are common and often cause no noticeable problems. However, larger discrepancies may alter the way the body bears weight and moves during walking, leading to compensatory movement patterns and increased stress throughout the lower limbs and spine.

Signs and Symptoms 

Many people with a mild leg length difference are unaware of the condition and experience no limitations. When the discrepancy becomes more pronounced or causes compensatory movement, patients may develop an uneven walking pattern, limping, pelvic tilt, or reduced walking efficiency. Treatment depends on the cause, severity of the discrepancy, skeletal maturity, and the patient’s symptoms. Small discrepancies may require no treatment, while larger differences may be managed with physical therapy, shoe lifts, orthotic intervention, or, in selected cases, surgical correction.
